Service Coordinator

The primary role of the Resident Service Coordinator is to help residents successfully age in the community and to advocate on their behalf. Our Resident Service Coordinator can provide access to supportive services in the community, offer support as needed and requested, and develop programs that support wellness.

The ultimate goal of the Resident Service Coordinator is to:

  • Empower residents to live as independently and self-sufficiently as possible
  • Respect the right of self-determination
  • Prolong ability to live in housing
  • Prevent or delay institutionalization
  • Provide monitoring for frail residents
  • Meet both individual and group needs
  • Combines coordination, advocacy, and skill building

In a nutshell, residents decide what assistance or information they need and with the Resident Service Coordinator figure out what is right for them. The things our Resident Service Coordinator can do for residents include, but are not limited to:

  • Financial Issues – Everyone can use some financial relief once in a while, especially if it feels like a struggle to pay for rent, food, prescription medications, telephone service, housekeeping, or personal care. There are many programs that can provide financial relief.
  • Service interventions – Help is offered with negotiating with public and private programs with which residents do business. Assistance with applying for and sorting out problems in the delivery of these services is provided.
  • In-Home Services – Whether it’s a housekeeper, a personal care aide, or delivered meals, a little extra help can make a huge difference in one’s quality of life. Affordability is always a part of the discussion.
  • Problem Solving – Questions about or problems with health insurance, medical bills, Social Security, phone service, or other consumer issues can be overwhelming. So can a resident-to-resident or resident-to-management conflict. A collaborative approach is used to find the solution.
  • Clinical interventions – Residents who feel lonely, are experiencing sadness or grief, or are suffering depression or anxiety sometimes need someone with whom to talk. The resident and the Resident Service Coordinator explore what’s going on to figure out the best way to get help.
  • Advanced Care Planning – Discussions occur with residents and their loved ones about life care planning (health care proxies, advance directives, DNR and MOLST orders, power of attorney, wills, etc.). 
  • Family Support – Direct assistance to families is offered by providing family-support services, caregiving support, information about useful technologies, and facilitating discussion with various care providers.
